Addressing C-F bonds and amyloid-formation in biological systems

The ingestion, pulmonary inhalation, and dermal infiltration of C-F bond-containing compounds, most commonly found in the form of per- and polyfluoroalkyl organic acids, causes oxidative stress, inflammation, DNA damage, and developmental defects in infants and adults. These chemicals accumulate in the brain, disrupt neurological function and compromise cognitive and locomotory behavior. Yet, we lack a high-resolution road-map of the interactions between C-F bonds and biomolecular assemblies driving the trajectory towards neurodegenerative outcomes. This gap constitutes a significant barrier to advancing measures designed to mitigate C-F chemistry-associated neurotoxicity. Emerging experimental and computational data from our laboratory reveals that perfluorooctanoic acid, perfluorodecanoic acid and perfluorosulfonic acid corrupt biomolecular structures through C-F:side-chain interactions in tested soluble, globular proteins found in milk and tissues (matrices where C-F chemistries have been detected). Furthermore, they impaired the physiological function in these proteins through displacement of physiological ligands or by compromising the binding of co-factors. The neuroblastoma-derived SHSY-5Y cell line insulted with the said C-F moieties displayed altered gene expression corresponding to reactive oxygen species (ROS), protein ubiquitination, inflammation along with compromised cytoskeletal integrity. C-F bond ingestion ablated dopaminergic (DA) neurons in the nematode C. elegans and induced locomotory deficits in a manner mimicking paraquat. Based on these findings, we propose to gather data towards our hypothesis that C-F bond exposure perturbs biomolecular, cellular and organismal assemblies to onset neurodegeneration-linked trajectories. In Aim 1, we will determine whether organic fluoroacids alter mRNA levels in differentiated SHSY-5Y cells and in neuroprotective gut bacteria (Lactobacillus rhamnosus, Bifidobacterium lactis and Lactobacillus acidophilus). We will examine whether the neuroblastoma cell line exposed to C-F chemistry displays readouts designed to inform the onset of neurodegeneration-associated trajectories (including α-synuclein aggregation). In Aim 2, we will further address in a preclinical model whether C-F burden induces protein aggregation (α-synuclein, amyloid β, mHTT), interferes with dopaminergic neuronal assembles and induces locomotory deficits. Completion of the proposed work will complement ongoing experimental biophysical, structural (crystallographic, NMR) and computational (docking, molecular dynamics simulations) mapping of the interactions between these anthropogenic “forever” chemicals and amyloid-forming proteins potentially resulting in a soluble-to-toxic transformation. It will prepare the stage for vertebrate testing. The findings from this relatively understudied area likely exposes interventional targets for C-F chemistry associated neurotoxicity, spurs therapeutic efforts and can also guide the development of more biocompatible alternatives.

Addressing disparities in pediatric intestinal failure

The proposed K23 Career Development Award will enable Vikram Raghu, MD, MS, to establish an independent research career focused on improving outcomes in pediatric intestinal failure. Pediatric intestinal failure is a rare, chronic, life-altering condition that requires many children to depend on daily infusions of parenteral nutrition to survive. Complications include life-threatening sepsis events, progressive liver disease, and venous thrombosis, with mortality estimated at over 10% by six years. Despite advances in care, outcomes vary widely, and emerging evidence suggests that geographic and socioeconomic factors strongly influence survival, healthcare use, and quality of life in this population. To address this knowledge gap, Dr. Raghu has assembled a multidisciplinary team of mentors and designed a training program in health disparities research (with a focus on socioeconomic and geographic drivers), multicenter health utility measurement, patient preference elicitation, transplant policy, and leadership in research conduct. These skills will support his overarching goal of optimizing intestinal failure management through decision sciences. The central hypothesis of this project is that variation in outcomes is partly explained by differences in geographic and socioeconomic conditions, such as access to healthcare, school resources, and environmental context, independent of clinical severity. To evaluate this hypothesis, he will accomplish the following specific aims: (1) Identify the effect of the Child Opportunity Index on survival and healthcare utilization in pediatric intestinal failure; (2) Examine socioeconomic and geographic differences in quality of life among children with intestinal failure using the Child Health Utility 9D; and (3) Determine patient and caregiver priorities for addressing socioeconomic and geographic barriers to care through a modified Delphi process and discrete choice experiment. By successfully completing these training objectives and research aims, Dr. Raghu will be positioned to become an independent investigator with the expertise to integrate decision modeling, disparities research, and patient-centered methods to improve outcomes for children with intestinal failure.

Addressing ethical challenges in whole genome sequencing implementation through human-centered design and ethnography

This is a K01 award application for Dr. Peter Taber, a sociocultural anthropologist and clinical informaticist to address ethical challenges related to secondary findings in whole genome sequencing. A K01 award will provide Dr. Taber with time to gain expertise in four areas: i) ethical, legal and social implications of pediatric genomic medicine; ii) healthcare partner-engaged implementation science in genomic medicine; iii) clinical informatics and health communication approaches in genomic medicine suitable for all populations; and iv) research leadership and management. These skills will enable Dr. Taber to meet his career goal of becoming an impactful independent investigator using social science and informatics to address ethical, legal and social implications of genomic medicine. Dr. Taber is supported by an interdisciplinary NIH R01-funded mentoring team, including: Dr. Guilherme Del Fiol, an expert in health informatics; Dr. Martin Tristani-Firouzi, a pediatric cardiologist and expert in pediatric genomic medicine; Dr. Sara Knight, a clinical psychologist and leading expert in engagement methods in genomic medicine; and Dr. Kimberly A. Kaphingst a leader in health communication in genomic medicine. Dr. Taber is also supported by advisors, including clinicians, genetic counselors and a bioethicist working with the whole genome sequencing program of interest. Whole genome sequencing is now a guideline-recommended first-tier test in pediatric medicine. Compared to conventional genetic testing, whole genome sequencing has a higher likelihood of returning findings unrelated to the indicator condition (“secondary findings”). Real-world guidance for ethically managing secondary findings is limited, because relatively few clinical whole genome sequencing programs have been implemented. Management of secondary findings that fails to recognize patient communication needs, cultural orientations and resource constraints risks exacerbating poor health outcomes. Addressing these challenges requires approaches that are context sensitive, and that engage healthcare partners. This project uses ethnography (the study or people in context) and human-centered design (design processes that emphasize input from end-users) to address secondary findings in whole genome sequencing. In Aim 1, ethnography will be used to understand families’ lived experiences with whole genome sequencing and receipt of secondary findings. In Aim 2, Healthcare Partner Panels consisting of healthcare staff and families will be used to design interventions and implementation strategies addressing secondary findings. In Aim 3, a prototype tool for patient education about secondary findings will be created. In addition to the general population, each aim will include two groups at risk of poor health outcomes in Utah: Spanish-speaking and rural residing families. A future National Human Genome Research Institute R01 will test strategies designed by healthcare partners (Aim 2), and a tool for patient education about secondary findings (Aim 3).
