Increased vulnerability to ferroptosis in Niemann-Pick disease type C

NINDS - National Institute of Neurological Disorders and Stroke

Niemann-Pick Disease Type C (NPC) is a fatal pediatric neurodegenerative disorder caused by mutations in NPC1 or NPC2, affecting fewer than 200,000 individuals globally. There is no cure or disease-modifying therapy approved for NPC in the United States, and further research to elucidate pathogenic mechanisms remains a high priority for effective therapeutic development. Despite well-characterized cellular pathologies, including endo-lysosomal enlargement, mitochondrial damage, iron and calcium dysregulation, lipid peroxidation, and chronic inflammation, how these diverse pathologies converge to drive disease progression remains unclear. This gap hampers the development of disease-modifying therapies. We hypothesize that NPC neurodegeneration arises from chronic cellular stress that culminates in ferroptosis, an iron-dependent cell death pathway driven by lethal lipid peroxide accumulation, and further characterized by lysosomal dysfunction, mitochondrial damage, iron and calcium dysregulation, and chronic inflammation. As a corollary, anti-ferroptotic drugs will delay or prevent the progression of this harmful process and, by extension, slow down cell death by this mechanism. Our preliminary data supports this hypothesis by showing: 1) elevated lipid peroxidation in NPC patient cells compared to sex- and aged-matched controls; 2) increased vulnerability of NPC cells to ferroptotic death, reversible by anti-ferroptotic compounds J147 and CMS121, which were developed by co-PI Dr. Maher, and 3) evidence of differential anterior-to-posterior ferroptotic activation in cerebellar Purkinje cells that mirrors the spatiotemporal pattern of neurodegeneration in NPC. We propose to test this hypothesis by pursuing the following Specific Aim: Characterize ferroptosis along the anterior-to-posterior axis of neurodegeneration in the NPC cerebellum and evaluate the ability of J147 and CMS121 to reduce NPC pathology and delay clinical symptoms in a mouse model of NPC. Significance: Successful completion of our study will help determine the extent of the contribution of ferroptosis to NPC pathogenesis and could establish ferroptosis as a therapeutic target, enabling repurposing of anti-ferroptotic drugs to slow neurodegeneration. Furthermore, both J147 and CMS121 already have Investigational New Drug approval so they could be rapidly moved to the clinic if proven effective in these studies

The NOFO strategies are to 1) SHARE - Formation of a network of AGS professionals to disseminate latest knowledge of AGS among stakeholders, including public health agencies, academic researchers, and advocacy groups, in order to improve patient care for AGS and the public health response. 2) EDUCATE - Create AGS communications targeted at the general public, AGS patients, or persons at higher risk for tick encounters (outdoor enthusiasts, dog owners, outdoor workers, etc.). Create communications targeted at healthcare providers including educational resources and training. 3) TRACK - Conduct active or enhances public health surveillance for AGS. Increasing Breadth of Immune Response by Single-Dose Influenza Vaccines

PROJECT SUMMARY / ABSTRACT Vaccines have been very effective at protecting against infectious diseases that pose serious threats to human health, but they can also be limited if the pathogens demonstrate antigenic drift, as is the case for influenza. This genetic drift causes mismatches between antigens in the vaccines vs. in the circulating strains, resulting in vaccines that lose neutralization potency against the new variants. Recent studies have shown that the release kinetics of vaccines can be important in establishing lasting and efficacious immunity. In particular, extending the exposure time to antigens can result in higher antibody titers and increase the breadth of neutralizing antibodies that target a broader range of epitopes (relative to conventional bolus vaccination). This can prevent the concern of low vaccine potency after viral mutations. To develop a single-administration vaccine platform based on these premises, we use a H5N1 avian influenza virus model, a pathogen for which the human population currently does not have existing immunity. We have shown that combining the effects of nanoparticles to effectively present H5 hemagglutinin antigen, together with slow release from a thermo-responsive PLGA- PEG-PLGA polymer depot to give extended antigen exposure, will elicit increased durability of the immune response, a broader cross-reactivity for viral variants, and protection from H5N1 infection. Many questions still remain, however, regarding the mechanisms by which this is accomplished and the optimization of this nanoparticle-hydrogel vaccine platform. Our specific aims are to: (1) determine optimized conditions for a nanoparticle-hydrogel vaccine platform and understand the contributing factors of their immune effects, (2) broaden cross-reactivity by incorporating antigens into nanoparticle-hydrogel vaccines that will generate homosubtypic and heterosubtypic responses, and (3) evaluate the cross-reactive and protective effects of combined, optimized vaccine elements. Because these vaccines are modular, and different antigens can be exchanged in a relatively straightforward approach, the successful implementation of this proposed strategy would have wider applicability towards the development of vaccines for other infectious pathogens and for universal flu vaccines.

Increasing kidney transplants with stratified kidney allocation: acceptability and impacts

In 2023, about 4750 Americans died while awaiting a kidney transplant (KT), but over 8,500 kidneys were recovered and not transplanted. The National Academies of Sciences, Engineering, and Medicine (NASEM) declared kidney non-utilization a critical problem that mandates immediate attention and remedy. Highly prioritized candidates receive many offers for superior kidneys, but also receive offers for many non-ideal kidneys that they refuse sequentially while the clock counts down to kidney non-use. Many candidates with low priority would benefit from receiving a non-ideal kidney quickly but rarely get these offers. Stratified kidney allocation is a proposed system under which patients would make an obligatory but modifiable choice between receiving a superior kidney after a longer wait or a non-ideal kidney sooner. Stratified kidney allocation might increase organ utilization while shortening wait times and improving transplant outcomes for selected patients. Prior work has shown that patients have misperceptions about the dimensions of kidney quality, underestimate the survival benefit of transplant with non-ideal kidneys, and alarmingly, are given a very limited role in decisions about offer quality. While stratified allocation might expedite and increase the use of non-ideal kidneys, impacts are unknown because myriad and sundry details such as which deceased donor kidneys would be categorized as non-ideal, which candidates would select which quality category, etc., have never been specified, nor have patient perceptions about acceptability been investigated. We will build expert consensus on the design of stratified kidney allocation, estimate the impact of stratified allocation to assist policymakers, and investigate attitudes and perceptions of patients, care partners, and providers on shared decision making about kidney offer acceptance, and on making a choice to receive either superior or non-ideal kidneys. Our aims are (1) To understand patient and care partner attitudes, perceptions, and preferences about stratified allocation, shared decision-making, and needs for transparency around organ offers and declines, (2) to assess transplant provider attitudes, perceptions, and preferences about a stratified allocation system, and (3) to estimate the impact of stratified allocation on kidney utilization, and to estimate waiting time for KT candidates choosing either to receive either superior or non-ideal offers. This research will take stratified allocation from a speculative proposal to a concrete policy design that supports deliberation and possible adoption, informed by the judgements of transplant experts, by patient and caregiver perceptions, and by numerical estimates of its impacts. Whether or not stratified allocation succeeds, we will, as advocated in NASEM’s 2022 report, investigate approaches for increasing transparency around offer declines, and involving patients in shared decision making about the kidney quality vs. wait time tradeoff.

Increasing the throughput of variant classification with multiplexed prime editing and single cell long-read technologies

PROJECT SUMMARY In genetic medicine, variants of uncertain significance (VUS) have posed a significant challenge. These variants, identified through genetic testing, are alterations in one's DNA sequence that cannot be definitively classified as benign or pathogenic. Approximately 80% of missense variants in the ClinVar database are classified as VUS. In clinical care, a VUS result frequently prevents clinicians from providing definitive diagnoses, limits treatment options, and causes uncertainty for patients and their families. Resolving VUS is thus essential to improve patient care in genetic medicine. Multiplexed Assays of Variant Effect (MAVEs) offer a high-throughput solution to VUS resolution by assessing variant effect for thousands of variants in vitro simultaneously. However, there are two limitations to realizing the potential of MAVEs to resolve VUS at scale. First, it is challenging to introduce a comprehensive set of variants into cells at scale, and second, MAVEs have historically relied on a small set of function assays, such as proliferation or survival-based assays, limiting the number of genes that can be investigated. This proposal aims to address both of these limitations, using CHD2 as a model gene, but developing techniques that could be applied to hundreds of clinically relevant genes. We will first advance high-throughput endogenous genome editing through the development of a suite of computational and experimental tools for prime editing. While prime editing offers a precise and versatile approach, it is plagued by low editing efficiencies. To address these shortcomings, we will develop a computational pipeline to design prime editing guide RNA (pegRNA) libraries with high predicted editing efficiency, an optimized cell line for editing, and a FACS-based approach to enrich for edited cells. We expect that such tools will boost prime editing efficiency for multiplexed cell libraries and thereby increase the efficiency of MAVEs. We will also address the limited set of functional assays for MAVEs. Here we propose scAVER-seq: a widely applicable and scalable single-cell Assay of Variant Effect using RNA-sequencing. This technique will combine transcript capture and long-read technology in single cells to identify each cell’s introduced variant and link it to its corresponding transcriptome in a single-tube workflow. By using the transcriptome as a functional readout, we will revolutionize variant classification in disorders associated with robust transcriptional effects, such as those caused by pathogenic variants in regulators of gene expression. This approach could help resolve the over 500,000 VUS that exist in gene expression regulators, which comprise 31% of all VUS in ClinVar. By developing generalizable methods to characterize VUS, we aim to contribute to the NHGRI's mission of understanding the impact of genomic variation on human health and simultaneously help deliver answers to families facing genetic testing results reporting VUS.

IND-enabling studies of non-immunogenic gene therapy for ultra-rare DMD patients excluded from dystrophin trials

Abstract Duchenne muscular dystrophy (DMD) is a rare childhood onset neuromuscular disease caused by genetic deficiency of the protein dystrophin. DMD is an incurable disease with a devastating impact on patients as a result of progressive weakness, wheelchair confinement, loss of independence/activities of daily living, and ultimately premature death from combined cardiorespiratory insufficiency. The majority of dystrophin gene mutations worldwide are multi-exon, frameshifting deletions that eliminate expression of variable portions of the 79 exon gene into the 427 Kd full length protein isoform Dp427. Dp427 is a cytoskeletal protein primarily composed of three protein folding domains, the largest of which is the rodlike mid-section composed of 24 spectrin-like triple helical repeats. Dp427 is thought to transmit muscle contractile force between the outermost sacomeres and the membrane-spanning proteins of the dystrophin-glycoprotein complex (DGC). Recently, an ultra-rare subset of DMD patients has been found to develop T cell-mediated, treatment-emergent serious adverse events (TESAEs) following systemic gene therapy using AAV vectors encoding miniaturized, ~ 140 Kd recombinant proteins derived from Dp427 by internal deletion. Our preclinical studies, as published in 2019 by Song, et al, Nature Medicine, predicted these TESAEs by virtue of our use of an informative animal model, the German Shorthaired Pointer Muscular Dystrophy (GSHPMD) in which a naturally occurring deletions eliminates peptide epitopes corresponding to the miniaturized versions of Dp427. In this study we also showed that a non- immunogenic alternative AAV vector encoding a miniaturized version of the dystrophin-related protein utrophin was protected from T cell destruction of transduced myofibers by central immunological tolerance towards this “self” protein. In this U01 proposal, we provide a milestone-driven, comprehensive translational and clinical research program to set the stage for a safe and effective alternative for the ultra-rare subset of DMD patients excluded from dystrophin gene therapy trials. We propose innovative, non-invasive physiological and biochemical outcome measures precisely targeted to the underlying pathophysiology of the disease, segmental myonecrosis incited by forceful muscle contraction. The four Specific Aims are structured to yield the highest probability of a successful IND application as the final deliverable, while providing ample information generalizable to therapeutics development for other genetic diseases under the NINDS mission. The proposed IND application will set the stage for a phase 1/2a clinical trial of AAV-microutrophin gene therapy in the ultra- rare subset of DMD patients with N-terminal dystrophin deletions, with results of the trial anticipated to apply broadly to the entire DMD patient population.

Indexed Exercise Blood Pressure and Its Relationship with Adverse Vascular and Ventricular Remodeling

PROJECT SUMMARY / ABSTRACT Increased arterial stiffness is a risk factor for heart failure with preserved ejection fraction (HFpEF), a disease that accounts for half of all heart failure, is twice in common in females, and once established has limited therapeutic options. Identifying individuals at risk for HFpEF before its development is therefore crucial. Arterial stiffening accelerates around menopause, which may represent an optimal time for intervention. While arterial stiffness is not commonly measured, exercise systolic blood pressure (SBP) may provide an opportunity to evaluate arterial stiffness and is assessed on every clinical exercise test. An exaggerated rise in peak exercise SBP above established cut-points is common (10-15%), but its presence does not drive clinical decisions due to conflicting data on its predictive ability for cardiovascular outcomes. This reflects fact that while exaggerated rise in peak SBP may be due to higher arterial stiffness, conversely it may also be a sign of health and reflect robust augmentation in oxygen consumption (VO2) during exercise. As the implications of an exaggerated rise in peak exercise SBP are unclear, our group and others have begun to examine how indexing peak exercise SBP to VO2 may better reflect vascular health. While it is well- established that females have lower exercise SBPs, we and others have shown that indexing change in SBP (peak-rest) to change in VO2 (peak-rest) unmasks higher SBP/VO2 in females versus males. No prior work has defined whether SBP/VO2, like arterial stiffness, accelerates around menopause and is associated with changes in cardiac structure and function that predispose to HFpEF. Because exercise SBP is collected on every exercise test but is not used in management, our goal is to define whether SBP/VO2 identifies those with high vascular stiffness at risk for HFpEF. To address this goal, we will combine 1) analysis of a large cardiopulmonary exercise test (CPET) registry (FRIEND) and 2) a prospective study using novel exercise cardiac magnetic resonance (CMR) techniques. In Aim #1, we hypothesize that indexing SBP to VO2 will unmask higher values in females than in males that will further diverge with age, specifically infecting upward at peri-menopause. We will establish how indexed SBP varies based on sex & age in FRIEND (n=10,000) and, in our prospective study (n=150) we will define how SBP/VO2 is impacted by reproductive vs. chronologic age, including longitudinal follow-up in females. In Aim #2, we hypothesize that in females more so than males, higher SBP/VO2, as assessed in our prospective cohort, will be associated with increased arterial stiffness, unfavorable CMR parameters, and biomarker profiles that are known to predict incident hypertension and HFpEF. We also hypothesize that higher SBP/VO2 will be associated with adverse cardiovascular outcomes as assessed in a cohort of heart failure patients in FRIEND (n=4,000). We anticipate that this work will facilitate use of SBP / VO2 to target future interventions to improve the disproportionate impact of HFpEF on females.
