PROJECT SUMMARY Temporomandibular joint osteoarthritis (TMJOA) is a degenerative condition that affects the temporomandibular joint (TMJ), leading to reduced mobility, malocclusion, and orofacial pain. TMJOA, particularly prevalent among women, significantly impacts quality of life and contributes to a substantial psychosocial burden. Despite its widespread occurrence, the mechanisms underlying TMJOA remain poorly understood, and effective treatment options are limited. A key factor associated with the severity of TMJOA is synovitis, the recurrent inflammation in the synovium result in angiogenesis, fibrosis or heterotopic ossification in TMJ. The objective of this proposal is to explore the role of perivascular stem/stromal cells (PSCs) within the TMJ synovium and their contribution to the regulation of TMJ homeostasis and the progression of OA. PSCs are multipotent progenitor cells located along the vessel walls and play critical roles in angiogenesis and tissue regeneration. Preliminary data from TMJOA mouse models suggest that PSCs exhibit sex-specific phenotype, with females demonstrating increased angiogenesis and differential PSC deposition pattern compared to males. These findings suggest that PSCs may contribute to the sex differences associated with TMJOA severity. This proposal aims to investigate the role of PSCs in TMJOA through two specific aims. Specific Aim 1 will evaluate the direct regulatory role of TMJ-PSCs in joint homeostasis and disease progression, using lineage tracing and trajectory analysis to track PSC activity, differentiation, and signaling pathways in a TMJOA mouse model. Specific Aim 2 will use in vitro functional assays and multi-omic analysis to assess the paracrine effects of PSC-derived extracellular vesicles (EVs), which have been shown to promote tissue regeneration by secreting bioactive growth factors and extracellular RNA (exRNA). This aim will involve in vitro and in vivo analysis of TMJ-PSC-EVs to determine their regulatory role during TMJOA and identify novel marker for therapeutic target. The results of this study will provide new insights in the sex-specific mechanisms driving TMJOA and identify novel biomarkers and therapeutic targets for TMJ degenerative diseases. By improving our understanding of the direct and indirect role of PSCs and their EVs in TMJOA, this research has the potential to lead to sex- specific treatment strategies, ultimately improving outcomes for patients with this debilitating condition.
