Project Summary To perform life-sustaining hemodialysis treatments, patients with End Stage Kidney Disease (ESKD) typically require their vascular accesses (e.g., an arteriovenous fistula [AVF]) to be cannulated with two needles thrice weekly to enable the flow of blood through a hemodialysis machine. As such, safe and effective cannulation is critical for successful hemodialysis treatments and is directly linked to patients’ morbidity and mortality. The cannulation procedure, however, is often fraught with complications due to small and/or deep veins, unusual vessel geometry, etc., that lead to infiltration and bleeding. These needle-related injuries can result in damage to, or loss of, the AVF, prolonging dialysis catheter use with all its attendant complications. As importantly, cannulation complications (including pain) rank very highly on the list of issues that are important to hemodialysis patients. Thus, cannulation is arguably the Achilles heel of vascular access in the $35B dialysis industry. Simulator-based objective metrics could be used to create standardized protocols for skill assessment and track an individual’s training and skill progression over time, even personalizing instruction to individual strengths and weaknesses. However, lack of standardized, structured and competency-based training for dialysis Patient Care Technicians (PCTs) and nurses is a limiting factor in today’s clinical care that results in hundreds of needle injuries every day, causing adverse patient outcomes and creating a significant financial burden (~$38K/major infiltration). Sojourn MedTech is developing CanSim simulator, a complete, smart trainer for learning cannulation skills that are critical for dialysis nurses and PCTs. The device will feature integrated hardware and software with realistic, vascular access models that are encountered in dialysis practice, and can be configured to use advanced, AI-based software models that deliver objective metrics-driven, personalized feedback for skill assessment and training. CanSim technology is based on more than seven years of extensive research on the original research prototype(s) of the device, with data collected from >80 novice and experienced cannulators. Results from our Phase 1 feasibility studies have demonstrated the validity of our core technology for quantifying cannulation skill on CanSim. The central objective of this Phase 2 proposal is to create and test a new, simulator-based Fundamental Skills training program on CanSim for hemodialysis cannulation skill assessment. CanSim technology will present the dialysis industry in the US a path to create structured, competency-based training and assessment for PCTs and nurses.
