About This Grant
This proposal aims to develop a novel treatment strategy for individuals with methamphetamine-associated congestive heart failure (Me-CHF) and methamphetamine-associated pulmonary arterial hypertension (Me- PAH). Methamphetamine use is a rapidly growing public health concern in the United States that disproportionately affects rural populations. The current rise of overdose deaths involving methamphetamine and other non-prescribed stimulants has been called the ‘fourth wave’ of the ongoing national overdose crisis. Chronic cardiopulmonary sequelae of methamphetamine use, such as Me-CHF and Me-PAH, are similarly increasing in prevalence and associated with significant morbidity and mortality. Hospitalizations related to Me- PAH in the US increased by over 9 times between 2008-2020, and annual inflation-adjusted costs related to Me- CHF hospitalizations in California increased 840% from 2008-2018 - totaling $390 million in 2018. Optimizing health for individuals with Me-CHF and Me-PAH requires both cardiology and specialty addiction expertise. Collaborative care models can facilitate this integrated treatment, supported by intensive care management and peer navigation. Moreover, collaborative care can provide infrastructure to develop and test novel interventions designed to increase care engagement and improve quality of life among people with Me- CHF and Me-PAH, such as using the evidence-based behavioral treatment Contingency Management to incentivize participation in a structured exercise rehabilitation program. To be maximally effective, collaborative care interventions must be adaptable to rural settings, which are most affected by methamphetamine and where significant geographic barriers limit access to specialty medical care and addiction treatment. To address this care gap, we propose to evaluate the feasibility and preliminary effectiveness of INSPIRE (INcreasing control over Stimulant use and Physical health through Incentivized REhabilitation), a 12-week remotely-delivered collaborative care intervention for rural patients with Me-CHF or Me-PAH featuring smartphone-based contingency management, remote synchronous exercise rehabilitation, and intensive collaborative care management. Our proposed Specific Aims (SA) are: SA1) Evaluate the feasibility and preliminary effectiveness of INSPIRE through a pilot randomized controlled trial of n=45 rural-living individuals with Me-CHF or Me-PAH, and SA2) Evaluate the implementation of the INSPIRE intervention using mixed methods, guided by the Practical, Robust Implementation and Sustainability Model (PRISM) and Reach, Effectiveness, Adoption, Implementation and Maintenance (RE-AIM) implementation frameworks. The proposed research is significant because it will develop and test a reproducible intervention to improve treatment access for all Americans with Me-CHF and Me-PAH. It is innovative because it will evaluate a novel addiction medicine-cardiology collaborative care program in one of the first intervention studies of any kind for this population. It is also unique in its simultaneous assessment of substance use-related, cardiopulmonary, and implementation outcomes.
