About This Grant

Abstract Placental blood flow analysis is complicated by having both maternal and fetal flow components. Using spectral analysis of the umbilical venous pulse wave spectral envelope, i.e., second order spectral analysis, we have been able to simultaneously assess maternal/fetal blood flow in the placenta, and we have shown preliminarily that normal and fetal growth restriction (FGR)/preeclamptic (PE) pregnancies can be distinguished. Early diagnosis and subsequent management of FGR and PE remains a significant clinical challenge. The ability to measure changes in placental blood flow safely and longitudinally, using a non-invasive ultrasound technique would provide early insight into pregnancies at risk and could be widely deployed in obstetrical care settings. These diagnoses would permit early initiation of preventative strategies and the ability to test therapeutic interventions to lessen the severity of disease in real time. This study will include normal gestations and gestations with FGR and/or preeclampsia. Umbilical vein pulse wave spectra will be acquired, and spectral envelopes identified as a function of time and analyzed by Fourier Transforms. Logarithms of the ratios of the maternal/fetal spectral peaks (LRSP) will be compared in normal and FGR/preeclamptic populations. Umbilical venous flow will be obtained by our established and standardized 3D color flow (3DCF) methodology. Our preliminary data using multivariate analysis shows significant population-based differentiation of the above conditions. In this proposed study we will attempt to reach diagnostic value, i.e., case-by-case differentiation, by means of these two specific aims: AIM 1. Determine the temporal characteristics of 3DCF and LRSP in a longitudinal study. AIM 2. Determine correlations of PE and FGR onset with cord blood flow and LRSP in a cross-sectional study If successful, this study will show that umbilical venous flow contains markers related to placental maternal/fetal blood flow, which can be used to assess FGR and preeclampsia. When coupled with umbilical cord blood flow, this new marker may potentially identify the primary causes of the two conditions. Our method is fast, safe, easy and could be quickly introduced into standard obstetrical ultrasound examinations.
