PROJECT SUMMARY/ABSTRACT In addition to reduced fecundability, women with obesity have an increased risk for pregnancy loss, and reduced live birth rates with fertility treatment, and increased perinatal complications compared to women with a BMI of 25 kg/m2 or less. Behavioral weight loss has not been successful in either improving fertility or reducing pregnancy complications. Therefore, other approaches are needed to address the reproductive deficit associated with obesity and to optimally prepare obese women for the healthiest possible pregnancy. We and others have characterized detrimental effects of female obesity on the hypothalamic-pituitary-ovarian axis, consisting of reduced luteal progesterone production and a decreased pituitary response to gonadotropin releasing hormone, a condition we have termed the Reprometabolic Syndrome. Work from our lab has shown that these key features of the Reprometabolic Syndrome can be induced in women with a BMI of 18-25 kg/m2 by administering a eucaloric, high fat diet (48% calories from fat) for one month. The current study proposes to test the hypothesis that the Reprometabolic Syndrome's endocrine features can be improved in women with obesity who consume a low fat (22% calories from fat), nutritionally balanced, eucaloric diet for one month. In Aim 1, we will evaluate daily urinary LH, FSH, estrone conjugates (E1c) and pregnanediol glucuronide (Pdg) in 30 women with a BMI of 30 kg/m2 or greater over four consecutive menstrual cycles before and after consumption of the low-fat diet to determine luteal progesterone metabolite production with the option for a long-term extension study. In Aim 2, we will perform early follicular phase frequent blood sampling over 3 hours in 24 women to determine LH and FSH baseline secretion and response to a weight based 75 ng/kg physiologic bolus of GnRH in the cycle preceding the low-fat diet and again in the subsequent cycle at the end of the low-fat diet. We will assess mean gonadotropin levels and response to GnRH. Aim 3 will examine metabolomic and microbiome profiles before and at the end of the low-fat diet to assess for salutary changes in free fatty acids, markers of mitochondrial function, oxidative stress and glutathione homeostasis, as well as microbial diversity, as a result of the dietary intervention. The potential for a relatively straightforward dietary intervention to improve fertility parameters in women with obesity would represent a readily scalable health benefit that could eventually be incorporated into an overall, evidence-based preconceptional health plan that will allow women with obesity to minimize risks to themselves, their pregnancy and their offspring.
