About This Grant

ABSTRACT Transplantation of deceased donor allografts presents significant challenges, including HLA matching restrictions and the associated risks of ischemia-reperfusion injury (IRI), precipitating long-term allograft dysfunction and graft loss. Here, we propose a novel, universal, cell-based immunotherapy using chimeric antigen receptor (CAR) Tregs targeted against intercellular adhesion molecule (ICAM-)1, a critical anchor protein in inflamed microenvironments upregulated in IRI and alloimmunity. By engineering Tregs with ICAM-1-specificity, including constructs coexpressing IL-10 to support a suppressive Treg phenotype, we will study the stability and immunosuppressive qualities of the engineered Treg products. Additionally, we propose methods for investigating the efficacy of these CAR Tregs in mitigating IRI in a murine heterotopic heart transplant model. The innovative line of studies described here seeks to (1) realize a targeted, HLA-independent CAR Treg therapy for transplant recipients and (2) address delayed graft function and long-term graft loss by offering an alternative to current chemical immunosuppressants that lack specificity and require lifelong treatment.
