PROJECT SUMMARY/ABSTRACT Breast cancer survivors (BCS) face a significantly elevated risk of cardiovascular disease (CVD), the leading cause of non-cancer death in this population. Obesity, a major and often overlooked contributor, significantly increases this risk beyond the effects of cardiotoxic cancer treatments. While the GLP-1 receptor agonist semaglutide shows promise for CVD prevention in BCS with obesity due to its substantial weight loss and its cardioprotective effects in individuals with established CVD, this has not been investigated. This study aims to address this knowledge gap by conducting a 24-week randomized controlled trial (n=50) comparing lifestyle modification alone to lifestyle modification plus semaglutide in BCS with obesity and without CVD. The study will comprehensively assess semaglutide's effects on preclinical markers of CVD, including vascular function and circulating biomarkers. We will assess vascular function using pulse wave velocity, augmentation index, and reactive hyperemic index (Aim 1), and evaluate circulating biomarkers using targeted proteomics via protein extension assay (Aim 2). Integrating vascular function testing and proteomic analysis will elucidate how semaglutide protects against CVD. This will in turn inform the development of early disease-modifying interventions to improve long-term outcomes for this high-risk population. This K23 proposal will support Dr. Hurtado Andrade's career development as an independent physician- scientist specializing in cardiometabolic health. Building upon her established expertise in clinical endocrinology and basic sciences, this five-year program at Mayo Clinic will provide focused training in three key areas: (1) a Master's degree in Clinical and Translational Science, establishing a strong foundation in clinical research methodology; (2) hands-on experience in the design and execution of clinical trials utilizing preclinical CVD markers; and (3) advanced training in proteomic and biomarker analysis. This comprehensive training program directly addresses critical gaps in Dr. Hurtado Andrade's expertise, equipping her to lead research on obesity- targeted CVD prevention. She will receive expert mentorship from a distinguished team: Dr. Chrisandra Shufelt (vascular health in women), Dr. Andres Acosta (obesity management and precision medicine), and Dr. Rickey Carter (clinical trial design and execution and bioinformatics). By integrating innovative methodologies with a robust career development framework, this project will inform the development of targeted CVD prevention strategies in cancer survivorship, ultimately improving long-term health outcomes for millions of women. In kipping with the NHLBI’s mission, the findings will contribute to personalized approaches to CVD prevention, enhancing quality of life and longevity for this vulnerable group. Moreover, this research will establish a foundation for a future R01 application focused on large-scale clinical trials designed to optimize cardiometabolic health interventions in BCS and other high-risk populations.
