About This Grant

PROJECT SUMMARY Alabama (AL), a priority state in the U.S. Ending the HIV Epidemic Initiative, requires substantial public health research to identify optimal HIV intervention strategies in support of greater HIV control. Among people living with HIV in AL, only 61% have achieved viral suppression in comparison to 65% for the U.S. average rate and far from the 2030 national goals of 90%. Better HIV control in AL can be achieved by leveraging public health data to identify and priority rank evidence-based interventions (EBIs) that are most effective and efficient. Particularly, mathematical modeling can be used to generate the evidence. Currently, the K01 candidate works with University of Alabama at Birmingham (UAB) investigators to implement a NIAID-funded R01 effectiveness-implementation trial evaluating three combination EBIs in the southwestern region of the state. While this trial will provide insight into clinical and implementation outcomes, it does not currently include plans to estimate the potential reduction in HIV incidence or impact of an expanded set of EBIs. Thus, the K01 candidate has an unmatched opportunity to embed mathematical modeling within this R01 trial and beyond, and engage with public health and other stakeholders in a participatory HIV epidemiologic modeling approach that will yield public health data that can guide state HIV intervention planning while advancing her career. Specific aims are: (1) To use mathematical modeling to determine the potential four-year impact of three EBIs to reduce HIV incidence (epidemiologic benefits) in the coastal region of AL. Guided by feedback from public health leaders and the R01 trial data, we will build a compartmental model to simulate the HIV epidemic in the coastal AL region and evaluate the three EBIs’ impact. Aim 2: To assess the impact of a) statewide scale-up of the three studied EBIs and b) expanded coverage to additional EBIs recommended by the Centers for Disease Control and Prevention (CDC). Guided by key informant interviews and focus group discussions with public health leaders, we will expand Aim 1 model to simulate the statewide HIV epidemic and evaluate the impact of selected EBIs from the CDC Compendium in reducing HIV incidence. Aim 3: To use participatory HIV modeling for the selection of optimal HIV intervention strategies for AL. We will conduct key informant interviews and focus group discussions with public health leaders using modeling findings from Aims 1 and 2. Through discussions we will inform public health leaders and facilitate consensus which will be followed by iteratively customizing analyses, yielding priority ranking of the EBIs for AL. By addressing the K01 aims, Dr. Nghiem will gain in-depth training and experience in participatory HIV epidemiologic modeling, including mathematical modeling, qualitative research, and dissemination science, positioning her to become an independently funded researcher.
