PROJECT SUMMARY / ABSTRACT Opioid use disorder (OUD) is a key driver of opioid overdose (OOD) and HIV incidence. Medications for OUD (MOUD) and pre-exposure prophylaxis for HIV (PrEP) reduce risk of OOD and HIV by 50% and >70%, respectively. Yet treatment initiation rates are low and retention is poor. Optimizing MOUD and PrEP use is crucial to improving OOD and HIV prevention. Health systems have an opportunity to close this treatment gap using biomedical informatics and implementation science. This proposal introduces LINK, a system-wide packaged strategy to optimize MOUD and PrEP utilization via 1) centralized telemedicine outreach to patients to increase MOUD and PrEP initiation (LINK2CARE), and 2) academic detailing for providers to improve MOUD and PrEP prescribing (LINK2EDU). An informatics-enabled patient registry underpins LINK. This registry summarizes up-to-date MOUD and PrEP utilization data enabling real-time LINK2CARE outreach to patients and real-time LINK2EDU performance feedback for providers. Thus, LINK uses EHR informatics to implement a system-wide “No Wrong Door” approach to improved opioid-related care thereby reducing missed opportunities to prevent OOD and HIV. LINK’s design is based on an integrated MOUD/PrEP care cascade cycle that recognizes the importance of initiation, engagement and re-initiation for improved MOUD and PrEP retention. While LINK2CARE enhances medication (re)initiation, LINK2EDU enhances engagement and retention. The LINK registry unites these components by tracking utilization across the full cascade cycle prompting outreach when patients exhibit treatment gaps to support (re)initiation, engagement and retention. This high-risk, high-reward proposal transforms health care for patients who use opioids by using informatics to pair system-wide patient identification with proactive outreach to offer integrated MOUD and PrEP services to untreated patients. Though lacking pilot data, this comprehensive groundbreaking approach carries high reward potential to expand and optimize MOUD and PrEP utilization thereby improving OOD and HIV prevention. By leveraging EHR informatics, LINK ensures high translatability to other health care systems including those without robust addiction services. I am a dual board-certified family and addiction medicine physician-scientist with a PhD in Population Health committed to using data science and community-based care to improve OOD and HIV prevention. My trajectory demonstrates my ability to pave innovative paths, develop new collaborations and skillfully execute research studies. This application marks a major step in my career by proposing a novel strategy with potential to transform healthcare delivery for people who use opioids and, in turn, strengthen OOD and HIV prevention nationwide. As such, this award will bring us closer to ending the OOD and HIV epidemics in the United States.
